Sundance, the CO of MLG, has said that their 360s have a 16% failure rate. That's from a pool of approximately 100-150 360s. This pool is probably more demonstrative of the typical 360 usage. They are used heavily one weekend per month in a quite chilly environment.

Under heavy usage strain, Rap's numbers seem about right to me.
